Primary Care Partners an affiliate of Atlantic Health System, seeks a Full-time or Part-time, RD - Registered Dietitian who is highly motivated to join our team in Northern/Central, NJ (may require some travel).

Job Responsibilities

• Performs individual nutrition and health assessments within Medical Nutrition Therapy guidelines.
• Interprets clients results and provides personalized lifestyle recommendations (diet, exercise, further lab testing etc.)
• Ensures nutrition counseling sessions for initial and follow up appointments entail: Motivational interviewing and Medical Nutrition Therapy diagnosis and recommendations.
• Proper documentation of each nutrition counseling session in the patients EMR (Epic) along with entering appropriate charge codes.
• Works closely with referring physician and their team to ensure appropriate coordination of care.
• Knowledge and proficient with virtual meeting technology (i.e. Zoom, etc.) and Microsoft Office preferred.

Minimum Education and/or Experience Requirements

• Bachelor's Degree in Nutrition and Dietetics preferred .
• Two-year clinical or outpatient experience preferred.
• Knowledge and proficient with Epic preferred.

Required Licenses, Certifications and/or Registrations

• Registered with the Commission of Dietetic Registration (RD/RDN).

Compensation is based on experience
