InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:
The REACH Community Health Worker (RCHW) provides engagement, care coordination/ linkage, and skill building to Hartford residents and their families navigating the healthcare system. The RCHW supports clients through an integrated approach to care management and community outreach. As a priority, the RCHW will promote, maintain, and improve the health of clients and their families by providing social support and informal counseling and advocating for individuals and community health needs. Community outreach , including home visits and health screenings will be required.
Essential Duties & Responsibilities:

  • Assists in providing each client access to necessary clinical, medical, social, educational, rehabilitative, vocationaland/or other services.
  • Collaborates with the clinical team and their clients to identify and coordinate required services by assessing theoverall needs of each client.
  • Provides outreach and linkages with various community resources.
  • Completes all required documentation related to treatment in accordance with established procedures and timelines including but not limited to IC documentation, referrals, court letters, gathering medical records, copying, faxing, scanning, collateral contact and appointment scheduling.
  • Establishes trusting relationships with community members and provides education, support and consultation to individual’s families and their support system which is directed exclusively to the wellbeing and benefit of the individual, if needed.
  • Supports clients to be actively involved in the service(s) he or she requires to remain in the community, arrange for needed services, regularly check on the client’s progress, monitor the impact of these services and advocate on behalf of their clients and reports information to referral agencies.
  • Follows up with clients via phone calls, home visits and visits to other settings where patients can be found.
  • Performs related duties as required including transportation of clients. Must have reliable and insured vehicle.

Requirements:Education &/Or Experience:
High School Diploma or Equivalent. Experience in a multi-cultural setting. Experience working in a setting attending to the needs of those in recovery from mental illness and substance use/addiction. Knowledge of some medical terminology and lived experiences as a community health center patient/ formally incarcerated individuals preferred.
