Gateway Recovery Center is looking for a full-time Recovery Continuing Care Coordinator. We are an inpatient, withdrawal management program (detox) in Inver Grove Heights. We offer high end, effective and state of the art detox services for clients throughout Minnesota. We pride ourselves on being empathetic, caring and compassionate. Our main focus is ensuring that our guest leave with a full continuing care plan at discharge.

We are looking to hire someone with a background in recovery community continuing care resources alongside great interpersonal and conflict resolution skills. With our guests being with us an average of 4-7 days, we focus on two goals - medical stability and an individualized continuing care plan. As the Continuing Care Coordinator you will work closely with the clinical team, guest and outside entities to ensure guest has all their needs met for discharge.

A. Clinical Procedures

1. Work closely with both the clinical and medical teams to ensure clients are set up with continuing care services based off of medical and clinical recommendations.

2. Ensure Release of Information (ROI) are in place for clients continuing care providers.

3. Make 3rd party calls, when ROI’s are in place to notify of continuing care appointments and coordinate treatment services.

4. Document meetings with clients, 3rd party calls and interactions pertaining to guest and/or continuing care coordination

5. Communicate on a regular and timely basis with other members of the clinical team.

6. Provide legible, written documentation of client notes/ progress in the EHR system and must be completed by program and DHS regulations.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
