Description

Our Endocrinology practice in Schenectady is looking for a Referral Coordinator to join their team! The hours are Monday - Friday 8:00 am - 4:30 pm.

This position is responsible for coordinating referral requests for patients and ensure patient records are received from external practices.

Essential Functions:
  • Track all orders placed in Allscripts. Ensure patient records are received from
specialty practice, radiology facility, or other healthcare professional and scanned
to the chart, adhering to PCMH and QI standards.
  • Index and categorize faxed and scanned documents in the Touchworks Scan
Application, and close out each order in Allscripts.
  • Utilize the CQS dashboard to ensure patient orders are accurate and complete.
  • Schedule outside appointments in a timely manner, relative to urgency, and
cancel/reschedule patients when necessary.
  • Perform necessary faxing of referral documents and patient medical records.
  • Follow up on older appointments for which we have not receieved a report
(database).
  • Document verbal conversations with patients when appropriate.
  • Document information into patient record in an accurate, legible and timely
manner.
  • Communicate with specialist offices and diagnostic facilities to set up necessary
appointments and testing for patients as instructed by the providers.
  • Follow up with patients by telephone if they haven’t completed recommended
testing and notify the ordering provider when testing is not complete.
  • Issue necessary referrals based on insurance requirements.
  • Obtain necessary documentation from referring physician according to insurance
requirements.
  • Obtain prior authorization based on insurance requirements.

Requirements

High School graduate or equivalent

Three years of administrative/clerical experience preferably in a medical office

Strong computer skills
