Job Summary: Provides professional case management services to individuals and families with the goal of promoting stability and self-sufficiency as part of the state-wide push to resettle newly arrived refugees. Utilizes a coaching approach to case management that is relational rather than transactional and connects clients to services and resources to achieve their self-determined long-term goals.

Salary Range: $21.00 - $30.00 per hour
Hiring Range: $21.00 - $27.00 per hour

Responsibilities:

  • Conduct comprehensive intakes and assessments with individuals and families immediately upon their arrival.
  • Develop, implement, monitor, and modify individualized service plans with clients based off their short and long-term goals.
  • Sign clients up for healthcare services and help schedule needed medical appointments.
  • Sign clients up for all eligible benefits through Colorado PEAK.
  • Set up a financial plan and explain benefit enrollments.
  • Refer clients to English language classes and follow progress through communication with instruction agency.
  • Refer clients to employment services and track their progress and satisfaction.
  • Enroll children of families into most culturally competent schools in districts where they find housing.
  • Input client information into databases as required.
  • Request financial approval for services of the program based on assessment of need and availability of resources. Maintain accurate case records; document client contacts and progress to ensure accountability.
  • Follow all rules and requirements of contractual Statement or Work with the Colorado Refugee Services Program.
  • Work with other staff to ensure proper completion of paperwork and adherence to grant criteria.
  • Assist with reporting as needed.
  • Collaborate with other refugee resettlement agencies.
  • Participate in staff trainings as required.
  • Communicate with volunteer “Cultural Mentor” teams for each family to follow families’ progress in this regard.
  • Adhere to all grant eligibility and documentation requirements.
  • Use creativity and innovation in program development and service delivery.
  • Maintain strict confidentiality of client and Agency information.
  • Other duties as assigned.

Work Environment:
This position will include remote work as well as a large amount of travel to transitional housing sites for refugee families, and to their permanent homes once housing is found. Mileage is reimbursed.

Work outside of regular operating hours required on occasion.

Requirements:

Education:
Bachelor’s degree in social work or closely related human services field, or equivalent combination of education and demonstrated experience. MSW preferred.

Experience:
Prefer 3+ years’ experience as a Social Case Worker for a public of private agency.

Other:

  • Knowledge and understanding of the Denver metro area resources and public benefits including TANF, Medicaid, SNAP, CHP+, CCAP, SSI, OAP, WIC preferred.
  • Knowledge of Refugee Cash Assistance programs a plus.
  • Strong communication, organizational and interpersonal skills.
  • Cross cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
  • Ability to interact effectively as a team member and independently.
  • Ability to multi-task while working with multiple client caseloads.
  • Must be proficient in Microsoft Office.
  • Must have a driver’s license, a good driving record, and access to own transportation.
