POSITION SUMMARY:

The RN Site Lead is responsible for the day-to-day Operation of a Regency Site. This position will assist the DON and Assistant DON in the day-to-day client care operations within the organization. This position reports directly to the RN Case Manager, ADON, and DON.

QUALIFICATIONS:

Have a current license as a Registered Nurse in the State of Minnesota and other states as indicated by agency Growth.

Home health care/public health, ventilator experience preferable.

Demonstrate integrity, good judgment, and initiative.

Ability to supervise and redirect other staff.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY:

Assistance in the Coordination of Services with RN Case Manager:

Role and tasks may include (but are not limited to):

Act as a Resource Nurse for all co-workers, especially new employees.

Make sure cleaning is being done.

Make sure appointments are made and follow up calls are completed.

Facilitate the 90 second huddle at the start of shift. (Create a plan for the shift make everyone aware of appointments, etc...)

Order medications.

Order supplies (client and office supplies)

Check treatment sheets every week and make sure tasks are completed. I.e.: catheter changes, trach changes, labs etc...

Be aware of long term needs of clients.

Be a “Positive” role model, inspire your co-workers

10. Function as a client advocate to promote client well-being

11. Look for any missed charting on MAR/Tx sheets and flow sheets every day and notify staff of any omissions right away.

12. Maintain chart order.

13. Check to make sure all MD orders are implemented and documented on MAR, pharmacy notified.

14. Measure all wounds 1x/week and ensure wound flow sheets are completed with every dressing change.

15. Participate in staff training, counseling, and disciplinary action as needed.

16. Participate in weekly case management updates.
