Registered Nurse - ACCS Team B (Leominster) Job at Open Sky Community Services
Description and Responsibilities:
The ACCS Integrated Team provides support and clinical/rehabilitative services to individuals with mental illness or dual diagnosis living in the community within North County. Our services focus on helping individuals transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The Integrated Team is comprised of peers, a housing coordinator, team leaders, outreach counselors, a nurse, clinicians, a substance use counselor, and a licensed clinical Program Director and Assistant Program Director. The team supports people living in group environments including Ashburnham St, School St, Highland Ave, and Catherine St as well as people living in their own apartments or homes.
Work days can be a challenge at times, however, with a pace slower than a hospital setting and working with people on a long-term basis allows for a unique opportunity for a nurse who is looking for a clinically based, supportive and nurturing role.
The schedule for this position is day shift Monday-Friday with on call coverage roughly every 6-7 weeks.
Key Responsibilities:
- Collaborate with the Integrated Team to complete a thorough physical assessment of health needs; provide referral to appropriate community physicians/health care providers as needed.
- Provide training and consultation to programs regarding medical issues, procedures and techniques, and health and wellness practices.
- Assess and provide individualized education related to person’s health and disease processes; reinforce wellness and preventative health measures.
- Monitor medical and psychiatric status by: observing and interviewing individuals, examining documents, and making recommendations.
- Participate in the development and implementation of the Treatment Plan as needed.
- Registered Nurse licensed in Massachusetts.
- Experience with adults with mental health and psychiatric disorders preferred.
- Excellent assessment, documentation, and verbal communication skills required.
- Valid driver’s license required.
