Provides, coordinates, teaches, or supervises nursing care for a group of patients usually in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patient's identified needs and priorities for care. Complies with all Home Care policies and procedures.

Essential Job Functions

• Performs comprehensive assessment of patient.
• Establishes a treatment plan that reflects the patient's diagnosis and goals as addressed in the assessment and physician orders, and adheres to the specified frequency and duration ordered by the physician.

Qualifications:

• Associate degree in Nursing required.
• Graduate of an accredited school of nursing, BSN preferred.
• Current licensure as RN in DC and willingness to become MD licensed.
• Current CPR certification.
• Valid drivers' license and current auto insurance, and working vehicle.
• Two years nursing experience required.
• Two years of experience in home care, critical care and emergency care preferred.
