The Registered Nurse (RN) Clinical Care Manager works closely with the Director of Clinical Care assisting in ensuring that all aspects of hospice nursing care and services are coordinated and managed appropriately.

The days and hours are M-F from 8:00 a.m. to 5:00 p.m. with some variability and some OT. There are weekend supervisory on-call duties which rotate with other clinical leadership to to provide telephonic support to on-call RN Case Managers and occasional in-person direct patient care should the need arise.

This is a salaried position which pays between $77,000.00 and $90,000.00 annually based on experience and internal equity.

The Clinical Care Manager works out of our office in Holland.

The Clinical Care Manager - RN Responsibilities (not all inclusive):

• May cover for the Director of Clinical Care in their absence.
• Assists with the supervision of all Clinical Care staff members, but primarily the Hospice Aides and Patient Care Desk staff..
• Assists the clinical services team to develop and achieve the strategic plan while continuing to identify opportunities for improvement.
• Assists with the evaluation of Hospice nursing care and services via the performance improvement program and productivity reviews. Promotes quality and safe delivery of patient care.
• Reviews and evaluates team competencies through a variety of means such as home visits, conferences, record and documentation review.
• Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course for their team as appropriate to ensure cost-effectiveness and regulatory compliance.
• Assists team clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care by attending interdisciplinary group meetings and case conferences as needed.
• Assists in the screening, selection, and orientation of new employees.

Qualifications for the Registered Nurse Clinical Care Manager include:

• Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan;
• Two years of recent clinical experience and one year of supervision is preferred. Homecare or hospice experience desired;
• Sound clinical judgment; and
• Knowledge and/or experience in pain symptom management with end of life care.
