This person is responsible for assisting physicians while providing exceptional treatment to patients with a variety of medical conditions.

The Heart Failure Program at Trinity Health of New England (includes Saint Francis Hospital, Saint Mary’s Hospital, and Mercy Medical Center) is looking for an energetic, experienced nurse to work primarily in our busy outpatient Advanced Heart Failure Centers.

This role is a critical component of the Advanced Heart Failure Clinic and will be involved in daily patient care to include, but not limited to, patient education, hospital transition visits, care coordination, remote patient monitoring and clinic IV infusions. The ideal candidate will be a self-starter and act as a liaison between the inpatient and outpatient Heart Failure Team to assist patients with transition from hospital to home.

This position will be a regional role sharing time between the Hartford and Waterbury sites.

Work Hours/Shift:

  • Full-Time

  • Monday – Friday

  • No Weekends or Holidays

You Will Be Responsible For:

  • Performs individualized nursing assessments, organize patient care and collaborate with staff and physicians to provide holistic patient care.

  • See post-acute patients for transitional care management, including disease management education, medication reconciliation, review of hospital stay and disease progression, self-management and symptom recognition, etc.

  • Assist in clinical education/orientation of other staff, both inpatient and outpatient

  • Guide and supervise assigned personnel in the delivery of patient care

  • Complete patient follow up as a part of the care team, including lab results, test results and required follow up critical value reports/monitoring

Additional Competencies:

  • Excellent interpersonal skills necessary in order to appropriately and accurately convey information to patients, physicians, APP’s, and other professional colleagues.

  • Confidence in ability to independently practice, including independent nurse visits with patients for medication management and titration, as well as education and medication reconciliation.

  • Must be able to troubleshoot patient needs via phone, appropriately triage and prioritize, and independently develop a care plan.

  • Excellent organizational skills required in order to effectively service all those involved in the management of our heart failure patient panel.

  • Ability to contribute to our Mission and Vision at Trinity Health Of New England as described in the job description.

Requirements :

  • Certification: BLS certification required

  • BSN with at least 2-3 years cardiology practice experience required; step down unit or heart failure experience beneficial.

  • Current RN license in the State of CT; graduation from an accredited nursing program
