Crossroads Counseling, Inc. is currently accepting resumes for a Full Time Registered Nurse. The Wellness Nurse will work to implement and maintain one of the strongest initiatives to be offered to the behavioral health system, A Behavioral Health Home. An essential component to the success of this program is to work under the leadership of a Registered Nurse. The wellness nurse will function as a lead health navigator, act as a consultant to other team members, and assist in coordination of the wellness team. In addition to the job description listed below, the nurse will need to travel between our sites in Northeastern PA including Williamsport, Lock Haven, State College, Sunbury, Bloomsburg and Wellsboro.

Major duties and responsibilities include:

  • Create global awareness of stakeholders throughout the behavioral health system on PH/BH issues.
  • Identify individuals who have extensive co-morbid PH/BH conditions.
  • Analyze individual and aggregate PH/BH care utilization data from PH/BH MCOs on these individuals.
  • Identify and provide, whenever possible through TCMs, outreach to individuals who are over using PH and/or BH services and offer them assistance in developing an individualized wellness plan.
  • Design and conduct periodic “health surveys” for individuals with co-morbid conditions.
  • Provide case-specific training and consultation to behavioral health staff (ACMs, D&A case managers, TCMs, PSPs, etc.) who are working directly with individuals with co-morbid conditions.
  • Develop and implement programs around compliance to medication and medical appointments.
  • When necessary, accompany individuals/TCMs to medical appointments to ensure they are getting optimal PH services and provide follow-up consultation and monitoring to ensure optimal compliance with PH providers’ recommendations.
  • Plan and secure Web-based and face-to-face trainings for BH staff and individuals on common health related conditions.
  • Advocates for the health of the individual by:
  • Referring and helping access to quality PH/BH services intended to meet individualized needs.
  • Facilitating access and coordination of PH/BH care services.
  • Being responsive to the needs expressed by the individuals themselves.
  • Assist and coach individuals to advocate for themselves in meetings with health care providers.
  • Conduct focus groups for individuals with BH conditions to flush out concerns and issues related to PH/BH coordination.
  • Develop a newsletter that focused on common PH/BH issues.
  • Develop wellness materials for individuals on identifying and managing chronic health conditions.
  • Convene team meetings for individuals with complex PH/BH conditions to ensure coordination of care.
  • Develop preventative wellness plans for individuals new to the system to preclude the development of chronic health conditions; i.e., weight management, nutrition, health screenings, exercise, and smoking cessation.
  • Communicates regularly with individuals, caregivers, families, county representatives, and local health care providers to establish priorities, identify training opportunities, and identify problem areas/needs.
  • Assists in educating provider network about the needs of individuals with SMI.

Job Type: Full-time

Pay: $42,000.00 - $62,000.00 per year
