Harbor Health also operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 450 frail elders who continue to live with dignity and independence in the community. To help further the mission of Harbor, we are looking for a talented Case Manager to join us at our partner site, Ann Lynch Homes in South Boston.

The Elder Service Plan (ESP) Resident and Wellness Coordinator is responsible for the case management needs of Elder Service Plan clients residing in Anne Lynch housing; as well as developing and running wellness programs for all residents within the Anne M. Lynch building. This position will assist ESP clients in navigating and accessing community services, resources, and adopting healthy behaviors. This position is located at the Anne M. Lynch Homes at Old Colony, 20 Rev. Burke Street, South Boston, MA. Position is 20 hours weekly, Monday through Friday, hours can be negotiable/flexible.

Role:

  • Establishes trusting relationships with Elder Service Plan clients living in the Anne M. Lynch Homes and their families, provides general support and encouragement, social support and informal counseling, and advocates for individuals and community health needs.
  • Provides support and referrals to local, state and federal agencies for ESP clients such as food stamps and The Ride.
  • Provides ongoing follow-up, basic motivational interviewing, and goal setting with ESP clients
  • Assists ESP clients to connect with transportation and provide appointment reminders in special circumstances.
  • Acts as a liaison between ESP Interdisciplinary Team members, ESP clients and Beacon property managers.
  • Works closely with ESP social workers and care team to help ensure that clients have comprehensive and coordinated care.
  • Records client care management information in the Electronic Medical Record (EMR) no later than 24 hours after contact.
  • Improves the quality of life for all residents within the housing development and promotes a positive sense of community among the population.
  • Coordinates and implements appropriate on-site educational and wellness programs for all residents.
  • Creatively sources and accesses new services and programs appropriate to the needs of all residents.
  • Exhibits excellent working relationships with clients, caregivers and staff, effectively communicating HHSI’s mission.
  • Attends regular staff meetings, trainings and other meetings as requested, occasional travel to other ESP sites for training and meetings as needed.

Requirements:

  • Bachelor’s degree in Social Work, Human Services or Related Field
  • Current BLS (Basic Life Support for Healthcare Providers) is required prior to hire
  • 2 years of professional experience working in community service-related field
  • 1 year of experience working with a frail elder population (paid or volunteer experience) preferred
  • Basic skills in effectively using Microsoft Office applications: specifically, Word, Excel and Outlook
  • Critical thinking skills to problem solve individually, and in a team setting
  • Bilingual desired, English /Spanish, Haitian Creole, Russian, and/or Portuguese
  • Ability to work independently and cooperatively as part of a multidisciplinary team
  • Ability to prioritize, organize and schedule busy workdays
  • Intermediate business writing skills
