Responsibilities:

• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff
• Implements specific restorative programs using the nursing process and ensures proper delivery and documentation with all nursing staff.
• Work closely with licensed therapists to ensure that specific recommendations are followed through when discharged from formal therapy programs.
• Performs other duties or functions as directed by the Director of Nursing

• Graduate of an accredited School of Nursing
• Must have a current state license to practice as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
• Current CPR certification
• Two (2) years experience in a long-term care setting preferred
• Background in Respiratory preferred
• Excellent communication and interpersonal skills.
