RN Care Coordinator Job at Lakewood Health Center
Overview
Work in the heart of #humankindess at CHI LakeWod Health in our Emergency department as well as assisting in our Medica Surgical unit! In this position you'll work full-time rotating day and night shifts, including every third weekend and rotating holidays.
As an ER RN Patient Care Coordinator at CHI LakeWood Health, now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, you’ll be the first touchpoint to the people in our community during health emergencies.
In our Rural Healthcare, Critical Access Hospital, you will utilize a variety of clinical skills and self-direction while being exposed to the emergency department, Acute Care, Swingbed,
Outpatient treatment and so much more.
To thrive as an ER RN PCC, you should exhibit leadership, teamwork, professionalism, critical thinking skills, and a willingness to take on additional projects such as patient experience initiatives. Responsibilities
What you'll do...
- Assist with staffing.
- Ensure policies and goals are achieved.
- Act as Supervisor and point of contact for staff nurse questions while Acute Care Manager is away.
- Use emergency triage protocol to intake, discharge or admit patients.
- Deliver stabilizing care to patients of all ages as part of the Acute Care team, adapting care to changes in patient’s condition as needed.

Qualifications
- Graduate of an accredited school of nursing
- Current licensure as Registered Nurse by the Minnesota State Board of Nursing
- Associates Degree in Nursing (ADN)
- Basic Life Support (BLS)
- ACLS, PALS, CALS & TNCC required or obtained within 12 months of hire.
