Fort Hudson Care Management, a program of Fort Hudson Health System, provides a wide range of client support through professional care management services. Our team members enjoy the support of a team-focused approach while working in an independent environment where their skills and experiences directly benefit our clients. Though effective care management, you will enable clients to remain in the community and delay or avoid long term care placement; and improve the quality of their life.
Requirements?
NYS RN licensure is required, and experience in community health roles certainly desired (but not required). The position requires travel to client homes (local); along with work from home.
