Family Care Health Centers is seeking a full-time experienced RN Care Manager (RN) with Primary Care/Community Health experience preferred to work as an integral role in our growing health care team environment. FCHC provides the highest quality of primary care services to its patients in a dynamic setting. FCHC also provides a culture of teamwork, professionalism and mutual respect. The RN Care Manager (RN) position works closely with designated care teams which includes residents and attending physicians. to plan and implement care, including patient education, care management, triage, medication reconciliation and other duties. The ideal RN Care Manager (RN) candidate must be enthusiastic, motivated, hard-working and flexible, be able to multi-task, have a great attitude, thrive in a fast-paced work environment and must be a rapid learner. The RN Care Manager (RN), in collaboration with the health care team, is accountable for assisting our patients toward their highest level of health. Position require to work a full-time schedule, Day Shift ~ Monday through Friday, and no holidays or weekends Will consider a part-time position, if necessary. Position reports directly to the Nursing Director. FCHC is currently operating under a situational mask mandate. ALL employees must meet COVID vaccine, FLU vaccine and PPD screening requirements.

Requirements:

*BSN preferred

*Active RN MO license and in good standing

*Experience as a professional RN required

*Primary Care/Community Health work experience preferred

*Bi-lingual skills a plus!

*Willing to work a full-time schedule; will consider a part-time schedule, if necessary

*Computer literate a must

*Ability to function effectively in a team environment with a positive/energetic approach

*Ability to communicate effectively verbally and in writing

*Ability to establish and maintain positive interpersonal relationships with patients and professionals of all disciplines
