This position is responsible for the direct and/or indirect care of patients assigned utilizing professional
nursing skills and knowledge. Analyzes the health status of the individuals and families in determining
nursing care needs, identifies goals and plans for nursing care. The position is responsible for ensuring
quality service is delivered to patients. The position supervises all RNs, LPNs within their territory and
provides second tier-problem solving and complaint resolution. In this position, you will maintain high
standards of ethics and integrity, including good communication, confidentiality, compliance with laws
and company policies, and professional conduct.
PRIMARY RESPONSIBILITIES
1. The RN Case Manager evaluates the effectiveness of the quality of nursing care provided.
2. The RN Case Manager collects complete and ongoing data to determine nursing care needs.
3. The RN Case Manager will participate in the development of an appropriate individualized care plan
based on age, cultural and spiritual values.
4. The RN Case Manager will complete the initial Patient assessment when necessary.
5. The RN Case Manager will document assessment/reassessment, plan, interventions, and
outcomes.
6. Adheres to infection control standards.
7. Follows clinical policies.
8. Knowledgeable in abuse recognition and referral. The RN Case Manager will act to safeguard the
patient from incompetent, abusive or illegal practice of any individual.
9. The RN Case Manager will participate in the development, revision and implementation of policies
and procedures designed to insure comfort and safety of patients in collaboration with other health
care personnel.
10. Ensures that patients receive quality care and that they are satisfied with the services they receive.
Resolves client complaints and problems, documents issues, and enters incident reports into
Enterprise Incident Management (EIM) as necessary.
11. Facilitates a team-based approach to program supports and services while practicing open
communication and respecting diversity.
12. Communicates regularly with management, providing status and activity reports as agreed.
Participates in staff meetings, agency sponsored in-services and team meetings as directed.
13. Completes all required reports and documentation.
14. Assists with the development and implementation of the individual's goals and objectives as
outlined in the Individual Support Plan (ISP). Assists the patient with attaining goals in order to have
positive outcomes as described in the individual’s ISP. Develops and oversees a team of
RNs/LPNs. Develops the schedule indicating which patients must be visited and when, ensuring
that the necessary follow up with patients is occurring as expected.
15. Provides training and orientation to new team members; Develops appropriate care plans for
patients; Monitors compliance with rules, policies, and regulations; Manages time tracking and
approves time-off; Communicates expectations and delivers constructive feedback.
16. Keeps abreast of relevant trends, rules, and regulations in the industry. Makes recommendations
to management for changes and improvements to enhance services or efficiency. Communicates
required changes to RN/LPN team.
17. Provide overall supervision and administration of Home and Community Based Services (HCBS),
including program development, implementation, and evaluation. Coordinate all personnel
activities, including the hiring, orientation, training, supervision and evaluation of program
employees and consultants.
18. Supervise and coordinate all aspects of programming, including preadmission and intake
procedures, admission, and compliance with HCBS plan requirements.
19. Resolve issues/complaints on behalf of individuals on IDD waivers to ensure satisfaction with
PAHM.
20. Responds effectively in emergency situations so that the client’s immediate needs are addressed.
21. Responds to emergency calls if/when they occur to ensure the safety of the individual. Provides
coverage in emergency and/or crisis situations.
22. Monitor patients’ progress and service delivery through on-site visits and data collection.
23. Travels to and participates in monthly team meetings, ISP meetings, Annual Review Updates, and
other team meetings, as necessary.
24. Respects the autonomy and right to confidentiality of all patients and families.
25. Advocates for changes in service(s) as necessary.
26. Performs all duties in an ethical manner and in compliance with local, State and Federal laws,
Medicare/Medicaid regulations and established personnel policies and procedures, including
confidentiality and HIPAA guidelines.
27. Assists in scheduling of RN/LPNs for each participant to ensure appropriate coverage for each
patient.
28. Performs other job-related duties as may be assigned.
COMPETENCIES
1. Knowledge and Experience. Current PA Professional Registered Nurse License, Bachelor of
Science in Nursing, Basic Life Support certification

4. Physical Demands.
a) This position requires the employee to work in a variety of environments (housing styles,
cleaning and maintenance, heating, and cooling) during visits to participants in their place of
residence.
b) Requires the ability to ambulate throughout a participant’s home, navigating stairs and other
physical barriers typical to private residences.
c) Must be able to deal effectively with stress.
d) Travel: In-Home and Community Based Services (HCBS) require the participant to actively
participate in community activities. Therefore, all Program Specialists must possess a valid
driver’s license, have clean driving history, and have access to a vehicle that is registered and
insured.
