P*POSITION SUMMARY*
Manages the day-to-day healthcare needs of independent Sisters of Mercy in the community to ensure their healthcare needs are met. Provides health counseling, nursing services, and acts as health liaison with the Sister’s Life Minister, social worker, and/or other health care providers. Ensure Sisters are treated with respect and dignity and ensure quality care as Sister’s healthcare needs change. Assists in securing an appropriate placement for a sister needing more care than her local community is able to provide. Responsible for the coordination of healthcare needs within the community and for health and wellness of all sisters residing in Vermont.

QUALIFICATIONS
· Registered Nurse licensed in the State of Vermont.
· Strong clinical experience, preferably in geriatrics and those with short and long-term healthcare needs.
· Commitment to holistic model of healthcare.
· Ability to communicate with others with a commitment to confidentiality.
· Ability to coordinate entitled services.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
TECHNICAL/FUNCTIONAL ABILITIES
· Sensitivity to Mercy charism/tradition.
· Ability to deal effectively with emergency situations, interruptions, stress provoking situations and varied personalities.
· Ability to communicate effectively with Sisters and colleagues.
· Appropriate knowledge of medical terms, procedures and processes.
· Ability to work as a team member, and independently.
· Possesses excellent written, verbal and interpersonal communication skills.
· Appropriate knowledge of medical equipment utilized including administration and monitoring equipment.
· Knowledge of HIPAA and general rules surrounding confidentiality and protected health information.
RESPONSIBILITIES
· Visit sisters who are hospitalized, in care centers, and at home and apprises Sisters Life Minister of conditions and concerns.
· Accompanies sisters to medical appointment as required and/or requested by sisters.
· Attends to the medical needs of all Vermont-area Sisters of Mercy including records, medications, treatments, lab work, doctor visits and emergency calls.
· Consults with hospital and/or physicians about hospital admission and discharge plans of sisters in the Vermont-area community.
· Regularly observe, monitor, assess, report and document sisters’ conditions and responses to treatment.
· Maintain accurate and current health records for continuity of care for all Vermont-area sisters.
· Provide support and information to sisters and their families when a sister is hospitalized or is in long-term care.
· Provide physical and technical care and support for sisters using appropriate healthcare resources.
· Updates Sisters Life Minister and Supervisor on the health of sisters and inform, as soon as possible, of any significant medical change in a sister.
OTHER FUNCTIONS
· Performs other duties as assigned by Supervisor requiring essentially the same skills, education and efforts as documented in this position.
· Participates, as appropriate, in personal professional development activities to maintain up-to-date knowledge of the requirements of the position.
· May be required to work extended hours including evenings and weekends, in special circumstances, or when planned ahead of time.
· May be required to travel to attend meetings, connect with Sisters, or similar.
Job Type: Part-time
Pay: $31.00 - $33.00 per hour
