Home Care RNs are responsible for client assessment, care planning, intervention and overall supervision of patient care and outcomes. Graduate of an approved nursing education program, licensure as a Registered Nurse in the State of Connecticut and have a minimum of one (1) year experience as a professional acute care nurse. Home care experience and accuracy in OASIS and developing a plan of care preferred. Waterbury, Hartford and Enfield, Full-time Days.
