Giving Home Health Care is a patient centered company delivering in home services to former DOE workers to ensure they are able to live safely in the comfort of their homes.

We are currently seeking an RN to start with a patient in Denver for 6 hours a week- you set the schedule with patient. Closest cross streets are North College and Martinez Park

We are a growing company with opportunities to increase hours if desired or just work with this one patient

* RN can take time to complete visit and get to know their patients while focusing on education and health management instead of paperwork and heavy lifting

* Dedicated care team to support you and the patients handeling all administrative task so RN is able to focus on patients clinical needs.

* Rate based on experience

Duties Include

Complete nursing assessment

• Complete full body assessment with special focus on the patients covered diagnosis
• Monitoring for changes in condition such as weight loss/gain, self-care abilities, indicators of disease progression
• Home assessments for DME needs
• Fall risk assessments
• Take Vitals
• Assess for indicators of infections
• Medication reconciliation & monitoring
• Provide education and training to patient, family and home health aide on disease process.
• Communicate with case manager and outside providers PRN to ensure there is a continuity of care
• Electronic charting
• Assisting a patient in being able to stay in their home
• Other duties based on patient care plan

Requirements : Current nursing License, must have a dependable and reliable transportation

Job Type: Part-time

Pay: $40.00 - $45.00 per hour
