Candlewood Valley is a 5 Star facility. Located in New Milford CT. We have 148 beds, with two long-term units, a secured memory care unit and a short-term unit. Become part of a premier provider of short-term rehabilitation, skilled nursing, and post hospital care.

Candidate must be a licensed Registered Nurse RN in the State of Connecticut with previous experience in the completion of resident assessments in a long-term skilled nursing setting.

RESPONSIBILITIES

  • The MDS Coordinator- Registered Nurse will oversee and facilitate the completion of resident assessments in accordance with Federal and State regulations
  • The MDS Coordinator- Registered Nurse will establish assessment schedules and ensure that assessments are done in an accurate and timely manner
  • The MDS Coordinator- Registered Nurse is responsible for transmitting assessments to the State of Connecticut, Department of Health
  • The MDS Coordinator- Registered Nurse will coordinate resident care plans in accordance to regulatory requirements, ensuring that resident care is delivered effectively
  • The MDS Coordinator- Registered Nurse will initiate Medicare coverage for newly qualified residents and sending denial letters when necessary
  • The MDS Coordinator- Registered Nurse will remain updated on changes in Medicare coverage and help determine documents necessary for reimbursement.

QUALIFICATIONS

  • Experienced Registered Nurse with knowledge of the MDS process
  • Must possess a Registered Nurse license in the State of Connecticut
  • The Registered Nurse must be detailed oriented with excellent communication and organizational skills
  • The Registered Nurse must have the ability to work cooperatively with our facility interdisciplinary team
  • The Registered Nurse will be flexible and decisive, with the ability to work under pressure
  • The Registered Nurse must have knowledge of state and federal regulations as they relate to long term care nursing reimbursement
  • The Registered Nurse has the ability to work flexible hours, when necessary
  • The Registered Nurse will communicate effectively with both a diverse clientele and workforce through sound business decisions, diplomacy, and discretion
