Schedule: Weekends only, 7am-10:30am

PRIMARY FUNCTION:

Provides direct and indirect patient care in the office setting. Completes patient triage and determines priorities of care based on physical and psychosocial needs. Communicates with physicians about changes in patient's status, symptomatology and results of diagnostic studies. Is able to respond quickly and accurately to changes in condition or response to treatment.

ESSENTIAL FUNCTIONS:

• Assesses patients/callers health status based upon presenting symptoms and follows defined protocols. Determines the urgency for care and directs patient/caller to the most appropriate level of care. Should access not be readily available at the practice, will direct patient/caller to an acceptable alternative venue of care. Seeks Physician/APC advice appropriately.
• In collaboration with Physician/APC, reinforces health education, counseling, and advice to patients and families on topics such as healthy life style changes. Effectively assesses patient educational needs, prioritizes patients learning needs, identifies barriers to learning, and uses various educational tools to encourage the non-adherent patient.
• Completes necessary paperwork, prior authorizations, and/or treatment plans
• Maintains patient records to ensure accurate documentation of the patient's medical treatment.
• Prepares and administers medications
• Documents and processes prescription requests and refills per Physician/APC order and or policy
• Processes medical records requests
• Interacts with co-workers, visitors and other staff.
• Other duties as required by the Outpatient Medical Director
  

Qualifications

EDUCATIONAL/TRAINING REQUIREMENTS:

ASN degree required. BSN preferred.

CERTIFICATES, LICENSES, AND REGISTRATION:

RN license to practice in the Commonwealth of PA.

EXPERIENCE REQUIREMENTS:

At least 2 years of experience in a practice or medical setting.

Conditions of Employment
