Registered Nurse/ RN Clinical Supervisor, Home Care
Location:
Hybrid schedule of in-office ( Brooklyn, NY 11220), field and remote
Salary range:
$102,000 – 125,000 based on education and experience; salary range maybe negotiable
$15,000 Sign-on Bonus
Position Overview:
Working in close collaboration with the Assistant Clinical Director supervises and manages all field clinical nursing staff in the provision of high-quality patient care in accordance with related agency guidelines, policies, as well as state and federal regulations. Facilitates the achievement of agency stated goals related to clinical outcomes, patient experience of care, fiscal sustainability and staff retention.
BSN, MSN preferred
Minimum of (3) years’ experience in a Certified Home Health agency. Supervisory experience preferred.
Knowledgeable in OASIS D; Clinical outcome measures; Federal Conditions of Participation and NYS DOH regulations related to Home Health Care; STAR ratings

- Oversees and evaluates the performance level of individual COCs, CHNs, LPNs, related to their ability to deliver, and accurately document, effectively coordinated quality driven and evidenced based care to clinically complex patients in accordance with the physician established plan of care. Creates an action plan in collaboration with the clinician and Assistant Clinical Director when indicated to improve clinician’s performance. Monitors clinician’s progress towards action plan goals and evaluates clinician’s ability to maintain improved and sustained performance.
