White Horse Village (WHV),a retirement community located in Newtown Square, PA is seeking to add
Home Care Agency Program Coordinator (RSSP) to our team! White Horse Village is an active senior living community offering a full range of living options and healthcare services. Responsibilities include:
- Schedule companions to provide home care services to WHV residents
- Ensure compliance with Department of Health regulations for PA home care agency
- Complete required documentation and monitor services provided
- Work collaboratively nursing/social worker to provide case management to residents
- Complete assessments/service plans
- Monitor the changing needs/health of clients and adjust service plans as need
- Work collaboratively with other health care providers to coordinate additional services
- Serve as an advocate for residents and families
- Develop and update departmental policies
- Perform quality audits
- Completes payroll and processes client billing
- Participate in recruitment, hiring and onboarding of new companions
Qualifications include:
- Bachelor’s degree in social work, human services field preferred; experience in working with older adults, and/or case management required
- Strong assessment skills and leadership experience
- Experience in home care, case management strongly preferred
- Experience working with older adults a plus
- Excellent organizational skills and ability to manage departmental schedule
- Vaccinated against COVID19 (will accept religious and/or medical exemptions)
- Have an up-to-date physical
- Have an up-to-date TB test Have an up-to-date Flu Vaccination for year 2022.
- Be able to pass a Pennsylvania Background Check
