Senior Care Program Case Manager Job at MAC Incorporated
- Must possess a minimum Bachelor's Degree in a human services field or be licensed Registered Nurse.
- Perform duties in compliance with all policies and regulations of the State Senior Care Program.
- Assess client for need of services with income eligibility.
- Develop care plans to fit client’s needs as resources allow. Revise as needed.
- Monitor services via home visits every six months, telephone communication every three months and more often when necessary.
- Allow clients access for contact during office work hours and voicemail after hours.
- Act as resource for referrals for her resources in the community a client may need.
- Maintain client files according to state policy in a secured environment.
- Weekly meetings with supervisor to discuss case issues, concerns.
- Comply with HIPPA regulations.
- Attend in-service trainings/conferences as available.
- Receive monthly bills from vendors and submit bills to comptroller for payment.
Job Type: Full-time
Pay: $19.50 per hour
