The Senior Market Medical Director (Sr MMD) will serve as a key member of the regional and market leadership teams. He/she will report directly to the Regional General Manager but also have dual reporting to the Regional Medical Officer. The Sr MMD’s primary responsibility is to serve as the key clinical contact for the client teams in the region and collaboratively develop strategies to manage the population of patients served by Landmark Health. This role will work very closely with the Regional General Manager, Regional Medical Officer, and the respective market leadership teams. This role will require travel throughout the region and the frequency will vary based on client relationship and market relationship maturity.


Primary Responsibilities:


Client and Community Relations


  • Build relationships with key client medical and operational leaders in the region to collaboratively develop strategies on population health management

  • Partner with internal client management team to demonstrate value to client partners through joint presentations such as joint operating committee meetings and program management meetings

  • Identify opportunities and develop strategies for the market leadership team to form community relationships with targeted hospitals, high volume PCPs, community acute care, post-acute care, long term care and assisted living facilities in the market

  • Serve as the clinical leader for presentations and discussions with potential new clients or expansion of existing clients in the region

  • Partner with other medical leaders in the Optum Home and Community Care pillar as relevant to the patient population within the region


Clinical Strategy & Standardization


  • Support the Regional Medical Officer as needed with development of clinical strategy based on client and internal priorities

  • Support the Regional Medial Officer with enforcing clinical standardization across the market

  • Oversee the standardized application of specific measurements of quality and ensure that the market teams consistently meet thresholds

  • Support implementation of national initiatives

  • Collaborate with shared services on special projects and company-wide initiatives on an as needed basis

  • Partner with the other market leaders to create and present at quarterly business reviews

  • Other duties as assigned by the Regional General Manager


Direct & Indirect Care


  • Provide consultative recommendations of the most complex patients within the population

  • Support clinical leadership to oversee quality and execute clinical operations within the market

  • Support the Regional Medical Office to provide the means, structure, and environment to most effectively utilize the resources that are available to ensure that all enrolled members have access to appropriate medical care

  • Participate in the engagement and management of high utilizer patients

  • Participate in the regional physician on call schedule and provide telephonic on-call coverage that require telephonic triage based on an as needed basis

  • Support the Regional Medical Office to drive market performance against key clinical and operational performance metrics

  • Remain current in the knowledge of geriatrics, utilization management, quality management, managed care principals and new clinical developments and technologies

  • Oversee and participate in the development, documentation, implementation, and interpretation of local medical policies and procedures

  • Participate in the continuous assessment and improvement of the quality of care provided to members

  • Participate in the resolution of medically related grievances

  • Interdisciplinary Team Collaboration

  • Collaborate with leaders in outreach, health services, pharmacy, dietary, behavioral health and others to ensure the highest quality medical care that meets internal and external expectations

  • Participate in weekly clinical leadership team meetings and other senior medical director meetings as necessary

  • Assist the Regional Medical Officer as needed in clinical education for the market staff

  • Support interdisciplinary team meetings, including case reviews that engage the full team, teaching on relevant clinical topics, and training on new protocols and workflows

  • Act as a local leader and advocate for the Landmark model with community providers and delivery systems and other community partners or potential business partners

Required Qualifications:


  • Physician currently licensed to practice without restriction in the state of practice and has met or exceeded all the credentialing requirements of a provider within the provider panel

  • Current in the knowledge of geriatrics, utilization management, quality management, managed care principals and new clinical developments and technologies

  • Experience in leading teams of physicians and mid-level providers in delivering high quality, cost effective care

  • Experience in value-based care models or fee-for-service models with strong focus on clinical outcomes.

  • Experience in working with diverse stakeholders on a common goal.

  • Experience in coaching members of the health care team to provide excellent care or, if needed, determine deficiencies in care and formulate a plan for improvement.

  • Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety Policy
