Senior Services Social Support & Meal Delivery Specialist - PART-TIME Job at Yellowhawk Tribal Health Center
Job Summary
This position is part of a multi-faceted system of support for the CTUIR elder and caregiver population. This position is funded through Administration for Community Living (ACL) COVID-19 Response grant award to Yellowhawk Tribal Health Center. The primary role of this position is to provide transportation to and from congregate meals at Nicht-Yow-Way Senior Center, deliver meals to homebound Elders and Caregivers, complete janitorial duties at Senior Center, and supporting the current workforce in maintaining access to elders and caregivers during the recovery of COVID -19 pandemic. The Title VI grant work at the Nicht-Yow-Way Senior Center includes driving individual elders and caregivers to obtain and/or access resources. This position will assist in the completion of this grant objective. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.
Schedule: Monday, Wednesday, Friday 10AM-4PM & Tuesday, Thursday 1PM-4PM
Essential Job Functions
- Deliver meals to homebound elders and caregivers per Title VI regulations with special attention paid to protecting this vulnerable population from influenza, colds and other communicable infections.
- Provide daily transportation to and from the Nicht-Yow-Way Senior Center for congregate meals to elders and caregivers ensuring social interaction and independence for mobile elders.
- Janitorial duties along with knowledge of OSHA/safety policies in regards to the work place/Nicht-Yow-Way Senior Center.
- Outreach to elders as assigned by Senior Center Program Manager.
- Keep confidential, accurate files, data and resources on Tribal Elders and their needs.
- Answers the telephone, greets individuals in person while using active listening skills to assess client needs. Assess needs of Elders/Caregiver/Grandparents raising children, facilitate referrals from other agencies or individuals and follows up with referrals to appropriate tribal, city, county or state resources that lead to assurance and empowerment.
- Maintain a positive working relationship with Yellowhawk, CTUIR, and Community Members.
- Maintain confidentiality in accordance with Yellowhawk Personnel Manual and the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Be sensitive to patients, but maintain professional boundaries.
- Complies with local traffic regulations to ensure the safety of the elders being transported.
- Must inspect Nicht-Yow-Way Senior Center vehicles weekly for preventive maintenance.
- Maintain vehicle log of elders and caregivers being transported, record mileage and vehicle maintenance.
- Other duties as assigned.
