Service Coordinator Job at Valley Vista MM LCC
Service Coordinator:
Assist and advise residents on available services that promote wellness and a self-reliant lifestyle
Educate residents and staff on available community resources (for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness)
Assist residents in creating informal support networks among themselves, with family members and friends
Monitor the delivery of services to residents to ensure they are appropriate, timely and satisfactory
Meet with service providers as needed and appropriate
Set up volunteer support programs with service organizations in the community
Maintain individual files on residents which will contain the following: intake information, service termination information, quarterly review and follow-up, resident/family meeting notes
Must possess time management skills and follow through in a timely manner
Keep up monthly calendar and help with newsletters
Excellent organizational skills and attention to detail
Ability to act with integrity, professionalism, and confidentiality
Maintain positive working relationships with co-workers
Part-Time (18 hours per week)
Pay depending on experience starting at $18 per hour.
