Social Services Care Coordinator @ Socorro

Basic Function

  • Coordinate clients’ ancillary health needs, focusing on clients’ primary medical care and financial assistance services.

Responsibilities

  • Ensure clients’ TB tests are completed within 7 days of admission
  • Apply clients for state disability benefits, as needed
  • Apply clients for Medi-Cal or My Health LA benefits
  • Arrange for client transportation to medical facilities
  • Assist clients to enroll with a primary care physician/medical group
  • Schedule new clients for physical examinations
  • Maintain professional rapport with all networking agencies
  • Act as liaison between clients and CDWs concerning medical issues
  • Act as liaison with Health Advisor for medication approval
  • Monitor clients ongoing medical issues and clients’ medications
  • Review all medical treatment forms when clients return to the facility
  • Report all client illnesses to Director of Clinical Operations
  • Inform the Program Manager of all client accidents, injuries and medical issues
  • Update and ensure accuracy of clients’ medication records
  • Document case management in Sage
  • Perform other related duties as assigned by the Director of Clinical Operations

  • Professionalism

Education/Training

  • If the individual is a registered addictions counselor, they must be enrolled in an academic Addiction Studies Program.

License / Certification Requirements

  • Position requires individual be registered and/or certified with one of the state-approved addiction counselor certification entities (i.e. CADTP, CAADE, or CCAPP) or be a masters level BBS registered individual.

Special Conditions

  • Must have a valid California driver's license. If a recovering addict, must have at least two years clean and be willing to submit to drug use detection testing.
  • Must be fully COVID-19 vaccinated, and boosted.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour
