Case Manager -Social Worker Assistant

Description:

Assist clients referred by NYC DSS/HRA to access services, benefits, and entitlements. The focus is to improve financial independence through employment or assistance obtaining federal benefits (SSI/SSD). Case Manager (CM) and client develop service plan & work together to help client achieve their goal(s). Staff work in the office and community Monday – Friday 8:30AM-5:00PM. Meet clients in -person and virtually. Connect clients with substance use treatment programs, medical, mental health, employment services, food pantries, rental assistance, childcare, etc. Collaborate and advocate with services to assist clients. Approximately 60% of work schedule is in the community.

Required

Spanish speaking Preferred. Ability to read/write Spanish a plus.

Community work in the Bronx, primarily in the south and central Bronx along subway and bus routes.

Office located in Wakefield neighborhood in NE Bronx.
