Social Worker/Counselor Job at Catalyst Health Solutions
Role and Responsibilities
To provide case management, counseling, and treatment planning with patients who are engaged in medication assisted treatment for opioid use disorder as well as poly substance abuse.
- See patients/clients in office individually or in group setting to assist in facilitating change, creating relapse prevention plans, and connecting patient/client to local resources to meet basic needs.
- Develop client treatment plans based on research, clinical experience, and client input as well as Review and evaluate clients' progress in relation to measurable goals described in treatment and care plans.
- Participate in multidisciplinary team meetings as well as supervision to facilitate growth.
- Comply with all HIPAA regulations, mandated reporting duties, as well as documenting progress appropriately.
- Coordinate care with courts, probation officers, community services and other agencies involved with patient.
Job Type: Full-time
Pay: $42,000.00 - $55,000.00 per year
