The Toe River Health District’s mission is to assure the conditions necessary for the residents of our service area to be healthy. To achieve this end, the District operates local health departments in each of its two counties, Avery and Mitchell. The local health departments accomplish the mission through an organized community effort focusing on health promotion, disease prevention, education and awareness, access to and provision of quality care and careful stewardship of public funds.

Position Summary

The purpose of Care Management for High-Risk Pregnant Women (CMHRP) and Care Management for At-Risk Children (CMARC) is to provide services to the high risk Medicaid population as defined by the program eligibility requirements.

The position includes access to electronic Protected Health Information (ePHI). Level of access to ePHI is determined by the job description and is set by the staff assigned to administer the specific software program that includes ePHI.

Essential Functions

  • Provide PCM services, proportional to the individual’s identified needs, to Medicaid recipients determined eligible after risk screening by a pregnancy medical home or pregnancy assessment by a pregnancy care manager.
  • Closely monitor clients’ pregnancies through regular contact with the physician and client to promote a healthy birth outcome.
  • Document all CMHRP services online in the Community Care of North Carolina (CCNC) Virtual Health system (VH).
  • Work with the local CCNC network and Pregnancy Medical Homes (PMH) to ensure program goals are met.
  • Visit Pregnancy Medical Homes (PMH), hospital, DSS, community organizations, and homes of clients and their families as needed to provide optimal service.
  • Follow guidelines in the Division of Public Health Agreement Addendum for PCM, DMA clinical coverage policy No.: 1E-6 effective March 1, 2011 for PCM, and all related trainings for PCM.
  • Provide CMARC services for all children birth to 5 years who have long term medical conditions, are in long term stressful situations, and/or are referred by the child’s doctor.
  • Identify children at greatest risk through data summaries and reports generated by the local CCNC network and utilization of risk screening data and provider referrals to area resources as needed.
  • All CC4C documentation is completed on line in Virtual Health (VH).
  • Provide service in PCP (Pediatric Care Provider) medical homes serving children within the county or serving residents of the county.
  • Care managers are responsible to educate patients, medical homes and community organizations of the benefits of the program.
  • Visit health care providers, hospital, DSS, community organizations, and homes of clients and their families as needed to provide optimal service.
  • Review annually and follow guidelines in the Division of Public Health Agreement Addendum for CMARC and all related trainings for CMARC.
  • Participate in emergency preparedness and other general public health training
  • Other duties as assigned by supervisor, including temporary assignments during public health emergencies

Qualifications

  • Social Worker with a bachelor or master’s degree from a Council on Social Work accredited social work degree program.
  • Two years of community medical social work experience and experience in managed care is highly preferred.
  • Registered Nurse: licensed by the State of North Carolina

License or Certification Required by Statute or Regulation: Is a license or certification required? What kind and type?

  • North Carolina Social Work Certification and Licensure Board bachelors or masters’ license
  • North Carolina Board of Nursing registered nurse license
  • Must possess a valid NC Driver’s License

Knowledge, Skills, and Abilities

  • Strong computer skills
  • Good organizational skills
  • Knowledge of general computer software systems to the extent that training on entering information into data collection programs can be accomplished in a few weeks.

Working Conditions

  • Prolonged periods of sitting at a desk and working on a computer. Physical demands include requirement to regularly sit; use hands and fingers; reach with hands and arms; talk and hear. The employee is occasionally required to stand and walk; lift up to 10 lbs.; vision abilities required include close vision, distance vision, depth perception, and ability to adjust focus.

Work Schedule

  • Work schedule is typically Monday - Friday 8:00AM to 4:30PM. Evening and weekend work may be required in special circumstances. This is a Fair Labor Standards exempt position meaning compensatory time is awarded on 1 hour for 1 hour basis for any hours worked in excess of 40 per week. Compensatory time must be approved by the supervisor prior to being earned.
