Job Duties:

  • Interpret and communicate overall organizational goals and apply them to team directives
  • Resolve escalated issues and remove hurdles preventing team from accomplishing medi-cal expansion and retention goals
  • Retain all active Families Together patients through the Medi-cal Redetermination process occurring in 2023.
  • Actively track and improve patient retention as whole.
  • Actively manage access to ensure maximum capacity for newly enrolled Medi-cal patients across all services.
  • Analyze all quality assurance measures related to patient acquisition and retention, determine areas for improvement, identify action plans and execute on strategies
  • Coordinate Medi-cal patient acquisition partnerships and opportunities
  • Monitor internal referral sources and conversion rates, collaborative with internal stakeholders to maintain appropriate levels of internal referrals
  • Develop and maintain policy and procedures relevant to these roles responsibilities and ensure standards of practice are met.
  • Complete all program requirements to ensure HRSA compliance management
  • Collaborate with the communications department to promote stories, articles, etc. related to medi-cal expansion and retention special project.
  • Participate in associations to remain abreast of policy changes, regulatory changes and practice update as it relates to the roles and responsibilities of this position.
  • Actively maintain patient roster lists by health network or assigned health insurance plan.
  • Research and implement industry best practices and trends
  • Facilitate communication and collaboration between teams and leaders within the organization
  • Contribute to huddles, newsletters, and other cascading messages
  • Ensure compliance with all workflows, policies and procedures
  • Responsible for success of assigned strategic goals, and action plans
  • Forecast and plan for future needs of the practice: space, specialty areas for growth, market threats, etc.
  • Role model the mission and core values of FTOC
  • Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
  • Researches and identifies opportunities for process improvements, provides analysis and recommends change strategies, and communicates proposed strategies.
  • Assists with planning, organizing and providing coordination, and leadership for special projects.
  • Identifies and communicates special project resource needs to both management and the internal teams effectively.
  • Under general supervision, assists with managing detailed project plan development and adherence to and tracking of status reporting/tracking of timelines, team meetings/scheduling for all deliverables associated with Medi-cal Expansion & Retention.
  • Produce, utilize, and ultimately become proficient with the electronic health record system and the ability to mine data from it.
  • Encourage or make recommendations on how this the electronic health record system can support primary care physician assignment and healthcare insurance assignment.
  • Interacts professionally with a diverse group, executives, manager and subject matter experts.
  • Works collaboratively with other team members in supporting multiple stakeholders.
  • Maintains current knowledge of all health insurance, health network, manage service organization and FQHC billing protocols and best practices in order to achieve success in this role.
  • Completes other projects and duties as assigned.

Educational & Experience Requirements:

  • High School diploma or equivalent required.
  • 4 years of healthcare experience required.
  • 2 year of project/ program management experience
  • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above is also qualifying.

Preferred Educational & Experience Requirements:

  • Bachelor’s degree in Public Health, Health Administration, Informatics, Business or related field
  • 5+ years of healthcare experience
  • 3+ years of project/ program management experience
  • 2+ year of experience occurring in a managed care setting (Health Plan or Provider Group)
  • 2+ years of experience using an electronic health record system

Job Type: Full-time

Pay: From $30.00 per hour
