VNANNJ is seeking a full-time (or per diem) Speech Therapist for a competitive paying, flexible position conducting therapy visits to adult and geriatric medical / surgical patients in the Home Health Care setting in Sussex and Warren Counties.

Responsibilities for a Speech Therapist in Home Health Care:

• Provides direct patient care throughout designated territory
• Develops and revises a Speech Therapy Treatment Plan based on client assessment and information provided through collaboration with the physician and other Agency personnel.
• Ongoing assessments to determine level of functioning including OASIS assessments at appropriate time points.
• Establishes long‑and short‑term goals.
• Evaluates outcome of care and planning for discharge.
• Secures orders for physical therapy every 60 days from the patient's physician. Medicare B is created every 30 days.
• Instructs other team personnel and family members in methods of assisting the patient to improve.

Qualifications for a Speech Therapist (ST) in Home Health Care:

• Master's Degree in Communication Disorders or Speech Pathology. Certificate of Clinical Competence in Speech Pathology or Audiology by the American Speech and Hearing Association.
• License by the State of New Jersey in Speech Pathology.
• Home health experience preferred but not essential.
• Experience may not be substituted for education
