Speech Therapist Home Health -Salt Lake County Job at Horizon Home Health & Hospice
Work with the physician and other staff in developing a plan of care for the patient and in updating the plan of care as indicated.
Provide services in conjunction with, or under the direction of a physician, providing patient assessment, patient care planning, direct patient care, teaching, supervision, in-service training, consultation, and administration of therapy.
Observe, record, and report the patient’s reaction to treatment and any changes in the patient’s condition.
Communicate frequently regarding the patient’s progress and condition.
Submit clinical notes and other documentation using the EMR daily
Submit initial assessment documentation along with proposed Plan of Care within 24 hours of the assessment visit.
Complete OASIS documentation when required within 24 hours of the visits.
Consult with the physician before changing treatment as outlined on the plan of care or increasing or decreasing visit frequency, and initiate written supplemental orders
Participate in patient care conferences and coordinate provision of services with other health team personnel, the patient, and the patient’s family as appropriate.
Meet all of the regulatory and licensure requirements and policies regarding personnel including, but not limited to, licensing, health surveillance, and in-service training.
