Substance Abuse Counselor Job at Gateway Healthcare
Summary:
Provides screening assessment referral evidence-based treatment and care coordination services on the Healthy Transitions Team (HT).This multi-disciplinary team is designed to provide outreach comprehensive evidence-based treatment referral and support services for a population of youth and young adults and their family members struggling with severe mental illness and co-occurring substance use. The goals of the Healthy Transitions Team Substance Use Counselor are: 1) To assist clients with substance use recovery; 2) Identify and provide intervention to overcome barriers to client recovery from substance use; and 3) Promote improve and sustain the overall wellness and physical health of the target population.
Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:
Conducts substance use treatment support referral and care coordination services with enrolled youth and/or families
Meets with Healthy Transitions Team staff members contributing knowledge and expertise to the team in case review and team planning
Support clients as they work towards their substance use recovery goals
Attend agency and Healthy Transitions staff meetings as scheduled.
Attend weekly supervision as scheduled.
Maintain detailed accurate and timely documentation in medical record.
Enhance and maintain professional expertise through educational activities and trainings.
Attend coordinated care meetings and report issues to Healthy Transitions Team Leader
Conducts screening assessments high-level care coordination and tracking of enrolled youth and their families to ensure follow up and connection with referrals made
Participates in trainings on Coordinated Specialty Care evidence-based wrap around models motivational interviewing cognitive behavioral therapy and practices and supported employment; Implements learning in day-to-day interventions
Participates in case consultation and multi-disciplinary review meetings to include integrated treatment planning of enrolled clients
Engages in training on Serious Emotional Disturbance Serious mental illness substance use disorders chronic disease and health conditions
Completes screenings utilizing standardized tools for Substance use and mental health including SBIRT trauma screening and tools for depression anxiety suicidal ideation and social determinants of health
Maintains a network of specialized providers to address needs of population
Ensures timely completion of GPRA outcome tool
Maintains standards for productivity and completion of clinical documentation
Follow agency policies and standards for client rights and confidentiality
Other duties as assigned

QUALIFICATION REQUIREMENTS:
Bachelors Degree in human services or related field; LCDP or higher credential required; Three years experience working in a community based behavioral health setting preferred; Experience in crisis intervention and the implementation of evidence-based practices preferred
NECESSARY SPECIAL REQUIREMENTS:
Valid driver's license and use of car during normal working hours or other means of transportation to complete major functions of job.
Automobile liability insurance coverage for vehicle used in accordance with center requirements.
JOB KNOWLEDGE SKILL AND ABILITY:
Understanding of mental health and substance use issues
Understanding of family systems model of care
Knowledge of community resources
Familiarity with screening/ assessment tools and evidence-based practices
Oral and written communication skills at a proficient level ensuring effective and professional communication
Ability to provide crisis intervention brief counseling or case management when determined necessary.
Ability to provide psychiatric/medical consultation to HT staff and/or community providers as appropriate to position.
Diplomacy strong collaboration and care coordination skills
Report writing abilities to include the formulation of clinical assessments and writing of monthly progress reports for grant purposes
Ability to track and aggregate data for outcome measurement and reporting
Ability to understand embrace and reinforce agency ethics and codes of conduct in all contacts with clients coworkers and the community
