GENERAL STATEMENT OF DUTIES:*
Under the direct supervision of the Suicide Prevention Program Team Leader, this person will be responsible for the program growth through referral source network building; developing lead-optimization strategies, and helping develop community engagement events to proliferate increased awareness, advocacy, and strategic partnerships with referral sources. This position will be focused on outreach to, and build strategic relationships with families, schools, hospitals, agencies and other organizations. He, she or they should have expertise in outreach activities aimed to connect our program with vulnerable youth and engage them in the program. Once enrolled, this staff member will continue to assist in providing community linkages. He, she or they provides supportive and proactive interventions to serve all clients experiencing mental health challenges by providing directly or providing linkages and who supports individuals and their families through difficult times and ensures that vulnerable people are safeguarded from harm. Their primary role is to help improve outcomes in people's lives. It is expected that this team member displays the utmost professionalism and integrity while executing the responsibilities below.
KEY ESSENTIAL FUNCTIONS:

  • Increase program and suicide awareness by educating community leaders, providers, referral sources, prospective participants/caregivers, and other audiences program benefits
  • Provide program education through lectures, presentations, events, meetings, or other necessary communications with community to facilitate referral generation.
  • Manage assigned strategic relationships, ensure continuous and effective communication with contacts, and respond to all inquiries, requests, and referrals timely.
  • Work with the Events and Marketing teams in identifying appropriate events, community partners, or other resources and opportunities to educate the community and provide education/technical/administrative support for planning and execution.
  • Perform necessary administrative functions of the program to include, but are not limited to, developing informational trainings, and putting together handouts/packets with approved marketing materials.
  • Closely follow and track all generated referrals. Consistently measure and evaluate individual performance and effectiveness, leading to continuous improvement in outreach efforts to maximize referral opportunities.
  • Directly assist in intake and provider relations as necessary, including introductory calls, general qualification screening, documentation collection, benefits education/applications, site tours, home visits, and other follow-ups to aid in the enrollment or of a prospect or family/client.
  • Regularly develop and plan a weekly and daily schedule of outreach activities in alignment with outreach strategies and targets as advised by management.
  • Maintain and generate reports to measure performance and quality of work set by the department.
  • Document all interactions and outcomes in the designated data management/electronic record system.

Work with the marketing team as needed in the development of promotional messaging and marketing collateral, as well as advising the team regarding public relations and other opportunities to increase awareness.

  • Build communication and relationships with clients, parents and caregivers, hospitals, schools, other outpatient clinics and treatment centers, youth offender programs, CPS, DSS, etc.
  • Conduct outreach and communicate regularly with clients, parents, support networks and other linkages
  • Is responsible for strengthening community partnerships
  • Update the community resource directory as needed
  • Submit regular program reports, attendance sheets, and complete other administrative duties as assigned
  • The role will require significant multi-tasking in a fast paced, challenging environment
  • The role will require field-based work, on call coverage, and a willingness to work flexible hours.
  • Provides linkages and supports to individuals and their families
  • Duties as assigned by supervisor

EDUCATION REQUIRED:
B.A. degree or higher in social work, psychology, rehabilitation counseling or a related human service field.
EXPERIENCE REQUIRED / LANGUAGE PREFERENCE

  • Experience working with community resources, partnerships, clients and families
  • Must have a commitment to work from a strength-based and/or youth development perspective
  • Able to work as part of a multi-disciplinary team
  • Ability to work with diverse cultural, educational, social and economic families
  • Experience with diverse populations and age groups (children and young adults)
  • Bi-lingual is preferred
  • Demonstrated competency in clinical documentation, stage-wise treatment and motivational interviewing.
  • Experienced in suicide prevention strategies
  • Able to remain flexible and work as a generalist in addition to a specialist

COMPUTER SKILLS REQUIRED:*

  • Strong computer skills are required. The ability to use Microsoft applications including Word, Excel, and PowerPoint are critical. The knowledge or ability to learn software programs including use of data and trends to inform clinical practice
