services provided by company's direct care staff, coordinating and delivering clinical services,

standards governing the provision of Medicaid Waiver programs.

Responsibilities:
Perform in home assessments and evaluations to determine support and client needs.
Conduct regularly scheduled supervisory visits of clients.
Monitor personal care and companion care services provided by direct care staff.
Assist in educating direct care staff to ensure quality of care.
Assist in building referrals and referral sources.
Report any concerns/issues to Branch Manager
Maintains confidentiality regarding client information and all related documentation per
HIPPA guidelines
Requirements:
Licensed LPN in the state of Missouri
At Least 1 year experience in health/home care
Current CPR certification
Free from health problems that may harm the well-being of the client or employee
Valid Driver's License and Auto Insurance, willingness to travel to outlying areas
