Support Broker Services Coordinator Job at DMR-Western Region
The Department of Developmental Services (DDS) is seeking candidates for the position of Human Services Coordinator I. The Human Service Coordinator I is the primary contact for individuals and families served by the Department of Developmental Services and are expected to effectively represent the Worcester Area office by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department.
Duties and Responsibilities: (these duties are a general summary and not all inclusive.)
- Coordinates and facilitates the development of Individual Service Plans (ISP) for individuals on assigned caseload.
- Acts as a Support Broker for people on their caseload- orienting people to the Participant Directed program offered by the Department, developing and monitoring individualized budgets, reviewing satisfaction with these services, and providing education to the area office staff, and outreach to the community, about self-directed service options.
- Monitors the implementation of services provided by DDS funded residential, day and support program providers
- Ensures individuals on caseload receive appropriate services through arranging, organizing and coordinating the Department of Developmental Services funded and generic support services in response to individual’s needs, while maintaining communication and outreach with community services and other state agencies.
- Monitors the safety and well-being of assigned individuals, providing advocacy in human, civil and legal rights
- Monitors follow-through on team recommendations while maintaining individual’s records and relevant computer databases, as well as maintain regular and consistent contact with individuals, service providers and families.
- Arranges or provides crisis intervention for individuals as needed, assessing needs and making recommendations for follow up planning
Preferred Qualifications:
- Ability to develop and maintain a respectful, caring, tolerant, ethical, and empowering relationship with individuals with intellectual and developmental disabilities.
- Knowledge of self- direction concepts, commitment to person- centered practices, and experience in self- directed service delivery.
- Familiarity with the implementation of Massachusetts’ “Real Lives” legislation
Special Requirements:
- Must possess current and valid Class D Motor Vehicle Operator’s License
Please Note:
This position is located at the Worcester Area Office in Worcester, MA.
Schedule: Monday through Friday, 9:00am – 5:00pm, with Sunday and Saturday off.
Flexibility is required to meet the needs of the Area Office.
