Purpose of Your Job Position:
The primary purpose of your job position is to transport the facility’s residents to medical appointments or other appointments as scheduled by the facility in a safe and efficient manner and in accordance with our facility’s policies and procedures.

Delegation of Authority:
As Transportation Associate/Activity Assistant, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

DUTIES AND RESPONSIBILITIES:
Transportation Functions:
  • Ensure safe and timely transport of residents to various locations in the community and surrounding areas through use of appropriate equipment and established standards of practice.
  • Assist in arranging transportation to other facilities when necessary.
  • Arrange transportation for field trips when necessary.
  • Adherence to rules of the road in accordance with local, state, and federal laws, i.e., speed limits, traffic flow, safety, etc.
  • Secure residents into secured chair during transport to include use of physical restraint and/or positioning devices per individualized plans of care.
  • Choose and use appropriate placement of tie down straps and seatbelts to secure any type of wheelchair during transport.
  • Operate and consistently demonstrate safe van lift operations.
  • Ensure safe transfer of residents to and/or from transport van upon arrival/departure to and/or from physician’s offices.
  • Ensure residents are properly checked into physician’s offices with necessary paper work given to office staff.
  • If necessary, remain with resident until appointment is completed as directed and/or instructed by facility’s administrative and/or nursing staff.
  • Report any incident and/or event that may occur during transport promptly to the Administrator/ designee in accordance with the facility’s policies and procedures.
  • Report any vehicle accident that may occur during transport promptly to the Administrator/ designee and to the appropriate law authorities in accordance with the facility’s policies and procedures.
  • In resident emergencies, provide and/or obtain necessary assistance for residents, i.e. emergency medical services (911) as indicated. Notify Administrator/designee of residents’ emergencies promptly.
  • Complete necessary safety checks and records per the facility’s policies and procedures.
  • Complete necessary documentation and/or mileage logs per the facility’s policies and procedures.
  • Keep and/or clean transport van as directed by Administrator/designee.
  • Maintain resident confidentiality.
Administrative Functions:
  • Assume the authority, responsibility, and accountability of Transportation
Associate/Activity Assistant.

  • Coordinate activities with other departments as necessary.
  • Keep abreast of economic conditions/situations and recommend to the Department Director adjustments in activity programs that assure the continued ability to provide daily activities.
  • Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed.
  • Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service.
  • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such as required.
  • Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Executive Director/Administrator.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Executive
Director/Administrator.

  • Report any known or suspected unauthorized attempt to access facility’s information system.
  • Others as deemed necessary and appropriate, or as may be directed by the Life Enrichment Coordinator/Activity Director.
Education:
  • Must possess, as a minimum, a high school diploma or its equivalent. Must be a Certified Nursing Assistant.
Experience:
  • None required. On-the-job training provided.
  • Must possess a valid driver’s license with a safe driving record in the state in which the facility is located.
Specific Requirements:
  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the public.
  • Must possess leadership ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing activity services.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
