Introduction
CTSI is pleased to offer a sign-on bonus for this position of $1000. The selected applicant shall receive one-half the incentive in the first paycheck and the second half after successful completion of 6 (six) consecutive months of employment provided the employee remains employed. The Sign-on Incentive is not part of the base salary calculation and is considered taxable income. The Sign-on Incentive is not included in retirement salary calculations. A full copy of the incentive policy is available upon request.

The Confederated Tribes of Siletz is requiring motor vehicle operators to transport Tribal Elders and program participants to and from trips sponsored by the Elders Council. This position also may provide back-up for transportation for the Medical or other Departments. This On-Call position is not eligible for salary increase nor does this position qualify for benefits, except as required by law.
Requirements
A valid Oregon driver’s license, insurance, good driving record and eligible to drive GSA vehicles or buses.
Knowledge of: Knowledge and skill to operate motor vehicles or buses safely.* Experience in: Working with Elders. Ability to: Drive vehicles with both standard or automatic transmissions, and ability to drive multi-passenger vehicle. Perform the work of the position with minimal supervision. Keep vehicle neat and paperwork in order. Interpret instructions, directions, road maps, etc. Use any department software for all transports. Obtain CDL (Commercial Driver’s License) if requested. Participate in training and pass certification requirements as requested by Transportation Department. Lift and move up to 50 pounds. Adhere to the confidentiality of individuals and information as described in the CTSI manual and federal law. Other: *Must be reliable and dependable. First Aid and CPR card required or ability to acquire with 60-days of hire. Successfully complete New Transport Hiring Training within 10 days of hire. Must possess a valid Oregon State driver’s license, insurance and be eligible to drive a GSA vehicle. Knowledge and skill of how to operate motor vehicles or buses safely.
Duties:
The purpose of the Elders Transporter (On-Call) is to ensure safe and consistent transportation of Elders to events. This includes helping to load and unload luggage, wheelchairs, and walkers. Must be able to assist elders in loading and unloading from vehicles. Conduct pre- and post-trip vehicle inspections and safety checks, and report all findings to the Transportation Department. The CTSI Transportation Policy, as adopted by the Siletz Tribal Council Resolution #93-216 establishes criteria. Coordinate the client pick-up schedule, working with the Elders Council Coordinator. Participate in required transport and vehicle trainings as required by the Transportation Department. Ability to lift up to 50 pounds occasionally. Adhere to the confidentiality of individuals and information as described in the CTSI manual and federal law.
After the six-month probation the employee will be evaluated of his/her performance and at that time, it will be, decided if the employee is entitled to a regular employee status.

Conditions of Employment
