With advanced rooming and discharge protocols, the LVN can complete the following tasks and enter documentation in the electronic medical record

The specific duties for advanced rooming including pre-visit charting include:

• Identify the reason for the visit and help the patient set the visit agenda
• Perform medication reconciliation and reviews need for refills
• Update past medical, surgical, family, and social history
• Past medical history that includes a list of patient’s present and past medical conditions
• Past Surgical History – information on all surgical procedures the patient has undergone, including dates
• Family History – information about diseases and health conditions in the patient’s family.
• Social History – information on marital status, occupation, use of alcohol, tobacco or drugs
• Verify medication allergies
• Recent immunization date and dosage if any
• Update problem lists and prompt the clinical provider on pertinent interval history with emphasis on chronic conditions such as diabetes, hypertension, substance use disorders, behavioral health, mental health, cardiovascular conditions, malignancies, among others.
• Update the following
• laboratory and imaging test results ordered in previous visit
• hospitalization, emergency room visits or specialist consultations.
• Screen for conditions based on protocols
• Pend or order preventive services based on standing orders
• Assemble medical equipment, if needed, before the physician enters the exam room as needed
• Discusses a summary of the above with the clinical provider

During the visit, the LVN and clinical provider enter exam room, while the provider examines the patient, the LVN documents the provider’s dictated findings in real-time. The provider then diagnoses and crafts a treatment plan, which are entered by the LVN. As part of the advanced discharge protocol, the LVN shall complete the following tasks as and document such in the electronic medical record:

• Print and review an updated medication list and visit summary
• Review other after-visit instructions, such as home blood pressure monitoring or referrals to subspecialists
• Coordinate the next steps of care, such as scheduling future appointments and labs

This augmented patient discharge process will ensure that patients understand and remember their discharge instructions, leading to improved treatment adherence.

REQUIREMENTS

• LPN License (CA License ONLY)
• Graduated from an approved LVN program and passed NCLEX-PN
• Clinical support experience preferred
• Epic experience - REQUIRED
