The Role*

The Nurse is a key member of the Circulo Health team who cares for the health of our members as a way to ensure the success of the Health Hub. This is an onsite role in Rotterdam, NY and Clifton Park, NY.

What We Expect from You:

• Manage the nurse phone line to address member health needs, including symptom assessment, triage, meds inquiries, and provider consultations.
• Conduct intakes and assessments as needed for new members
• Document all member communications, orders, referrals, and medications
• Maintain professional communications with members and clinical team
• Provide RN oversight of the clinical team for in-office immunizations, medication reconciliation, and care coordination functions.
• Maintains a holistic, team-based philosophy

Degree Requirements and Certifications:

• Registered Nurse with active licensure in the State of New York
• Must have primary care, family practice or pediatrics experience.
• Minimum of 2 years of direct patient experience
• CPR certification required
• Outpatient/ primary care experience preferred
• Pediatric experience preferred
