SUMMARY:
The Admissions Coordinator is responsible for providing support to the Admissions Director with the admissions process and the marketing program. Responsibilities include assisting to maintain a positive relationship with hospitals, doctors’ offices, nursing homes and other related health care providers through caring communication and strong focus on customer service.

PRIMARY RESPONSIBILITIES:
  • Accurate completion and timely submission of Daily Census;
  • Accurate completion and timely submission of Bedboard;
  • Accurate completion and timely submission of Pending Report;
  • Updating Referral tracking list to reflect daily referrals;
  • Updating Admission Tracking List to reflect accurate admission information;
  • Performing daily Angel Rounds within assigned area with weekly submission of findings;
  • Ensuring Admission Packets are submitted appropriately to Patient Liaison;
  • Ensuring dashboard is updated with confirmed and/or potential admissions;
  • Ensure dashboard is free of admission errors;
  • Scanning medical file to shared drive as appropriate;
  • Reviewing Admissions packet for completion after receiving from Patient Liaison;
  • Assist to ensure a positive admission experience for resident and family;
  • Assist to maintain census goals, updated bed availability and facility services information by working with facility’s interdisciplinary team as appropriate;
  • Compile and distribute admission papers which includes communicating the admission criteria to referral sources, residents and families as appropriate;
  • Ensure that all papers are properly completed, signed and distribution of paperwork to appropriate individuals;
  • Meet with family and resident to assist with accurate completion of Medicaid applications and LTC as appropriate;
  • Examine building and rooms to ensure they are prepared for tours and admissions.
  • Ensure photo release has been signed by each resident.
  • Maintain a par level of 20 prepared Admission packets.
  • Maintain working knowledge of medical, social and financial information relating to elder care including but not limited to Federal and state regulations and Medicare/Medicaid regulations and reimbursements;
  • Assisting with expedition of admissions process for emergencies;
  • Work with nursing and housekeeping staff in maintaining a model room for tours;
  • Participate in meetings to alert appropriate staff members of projected admissions, room changes, discharges and appointments/tours as appropriate;
  • Communicate special needs of new admissions to staff to ensure a smooth transition;
  • Concerns him/herself with the safety of all facility residents in order to minimize the potential for fire and accidents. Also, ensures that the facility adheres to the legal, safety, health, fire and sanitation codes by being familiar with his/her role in carrying out the facility’s fire, safety, and disaster plans;
  • Always puts customer service first. Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights;
  • Serve as primary coverage of receptionist desk;
  • Performs other duties as assigned.
QUALIFICATIONS:
  • Minimum of one (1) year admissions experience in a health care setting.
