The Association for Mental Health and Wellness is a recognized leader in state-of-the-art rehabilitation and support services. We are currently looking for a qualified leader to assist in the day-to-day operations of our Adult Health Home Care Management Program.


Key responsibilities may include, but are not limited to:


  • Maintain a blended caseload; includes Health Home Plus members with Serious Mental Illness (SMI).
  • Proficient in conducting appropriate screenings/assessments (i.e. Comprehensive Assessments, Risk Assessments).
  • Coordinate Care Management needs and successfully engage high need individuals by utilizing a recovery-oriented approach and skills such as motivational interviewing and trauma-informed care.
  • Provide oversight to and monitor productivity of team of Care Managers.
  • Assist with the monitoring of team’s diligent search efforts to reengage clients.
  • Review/distribute reports and ensure that members of team are meeting service delivery requirements monthly.
  • Assist Team Coordinator with the oversight of special projects.
  • Monitor team’s completion/submission of documents (i.e. BSQ), assist in the tracking of inactive Medicaid list and follow up on status with team members.
  • Provide support to new team members as they navigate IT platforms (i.e. Foothold).
  • Identify performance issues/training needs and bring to Team Coordinator’s attention.
  • Participate in staff meetings; help to facilitate team meetings and other administrative meetings/trainings as required.
  • Perform duties with designated amount of time in community and office setting.
  • Respond to and supports Care Managers with urgent client situations in the office and community, as needed. Utilize crisis intervention skills and access community supports (i.e. Mobile Crisis, DASH) when necessary.
Qualifications:
  • Master’s degree in a human service/health discipline, supervisory and care management experience preferred; SIFI certification a plus.
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help. Experience working with those who have chronic physical health and/or behavioral health diagnoses (mental health and/or substance use).
  • Reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
  • Strong administrative and leadership skills; critical thinker with excellent written/verbal communication and active listening skills.
  • Computer skills and knowledge of computer applications such as Microsoft Excel; will be working in multiple IT platforms including electronic health record.
  • Bi-lingual, English/Spanish is a plus; stipend is awarded for assessed and verified bi-lingual proficiency
  • Candidate must be fully COVID-19 vaccinated.
