Thank you for your interest as a Therapist working with adults who are a part of our In-Home Team in Lawrence County. This position allows the team to improve the lives of individuals who need closer monitoring that they can receive in an outpatient setting. Please review the job responsibilities below.

DUTIES AND RESPONSIBILITIES:

· Function as the bachelor’s level staff member in a two-person treatment team providing home based services to individuals who refuse other outpatient services and/or who need temporary support due to increased symptoms or transition from a more intensive level of services.

· Key service functions include the following: individual or family counseling, crisis intervention, mental health consultation, basic living skills, family support and education, case management and medication monitoring.

· Needs assessment: (Examples of tasks: using required forms; determines through assessment clients assets, deficits and basic needs; reviews needs on regular basis.)

· Develops service plan. (Examples of tasks: contact provider source; write goals which may include personal grooming and health teaching; medication knowledge; financial aid; etc.)

· Implements plan by providing case management services. (Examples of tasks: monitor client condition; make home visits; assist client in securing needed services.)

· Represents the needs of the seriously mentally ill. (Examples of tasks: establish liaison with other community resources; identify service gap; advocate for client.)

· Participate in insuring quality of services. (Examples of tasks: participate in case management training; complete work in accordance with standards; participate in staff meetings; participate in peer review.)

· Basis Living Skills Instructor. (Examples of tasks: provides didactic, process, and experiential instruction to clients.)

Job Type: Full-time

Pay: $13.94 - $15.00 per hour
