POSITION SUMMARY

To provide superior quality, competitive value, and outstanding service by providing the highest standard of quality medical care to Brevard Health Alliance (BHA) patients through the primary care medical home model. Under the supervision of the Chief Medical Officer, the Advanced Clinical Document Manager (APRN, PA) assists in extraordinary patient care through remote desktop monitoring and management.

GENERAL EDUCATION REQUIREMNTS

Master of Science in Nursing from an educational institution approved by the Florida Department of Health.

Master of Science in Medicine in Physician Assistant studies from an educational institution approved by the Florida Department of Health.

ADDITIONAL QUALIFICATIONS

  • Clear and active Florida Advanced Practice Registered Nurse/Physician Assistant licensure
  • 3+ years’ experience of related clinical experience preferred. Experience in a primary health care environment, community health center, or Federally Qualified Health Center (FQHC) preferred
  • Experience working in various outpatient settings
  • Current Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
  • Exceptional skills in Electronic Medical Records (EMR)
  • Understanding of Quality Metrics (HEDIS/UDS)
  • Demonstrates strong oral and written communication skills
  • Ability to speak and understand the English language to communicate with patients effectively
  • Extensive knowledge in clinical management and population health

PRIMARY ACCOUNTABILITIES

  • Refill’s medications, as determined to be appropriate, but may be out of support staff’s scope of job duty (i.e., RN, MA)
  • Reviews phone notes, triages patients when appropriate, or relays messages to the other medical staff members to finalize patients’ requests
  • Reviews patients’ inquiries sent in the patient portal and responds, when appropriate and needed
  • Assists in Prior Authorizations when a clinician is needed
  • Participates in reviewing patients’ preventative measurements and ordering tests/follow up with results (i.e., mammogram, colonoscopy, etc.)
  • Demonstrates competent skills in therapeutic, cost-effective pharmacologic administration and management
  • Promotes patient/family-informed decision making in treatment plans and options
  • Emphasizes preventative care, disease management, and the importance of screening tests
  • Consults and collaborates with BHA clinicians when needed
  • Participates in continuous quality improvement efforts
  • Attends department meetings, provider meetings, etc. as applicable
  • Maintains internal customer satisfaction
  • Creates televisits with patients when applicable
  • Telephone on-call 1 week per month
  • Assists when providers are out of office to cover their desktops
  • Reviews outstanding labs/diagnostics and follows up appropriately
  • Assists in consultation with specialists for select patient cases
  • Coordinates response to STAT faxes, critical labs, etc.
  • Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s Key Performance Indicators and will be evaluated on a monthly and quarterly basis.
