SUMMARY: Provide primary care and maintaining continuity of care for patients of the community by establishing diagnosis and treatment of common acute and chronic illness with health promotion and disease prevention.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes the following, other duties may be assigned
Ensure quality healthcare to patients.- Examine patient / review medical history.
- Manage medical/health/ diseases.
- Review / prescribe medications.
- Order/ review additional test / labs.
- Order referral for patients requiring services.
- Order tests (labs, x-rays, MRI, etc.) review results in a timely manner.
- Review patient tasks for refills, care questions.
- Educate patient.
- Advise patient of the importance of participating in their health care.
- Review and advises patient with questions/ concerns.
- Educate patients to illness/disease verbally or with printed materials.
- After hour services.
- Participate in rotation of on –call assistance.
- Advise patients of appropriate after hour care.
- Manage daily/ weekly schedule
- Document in electronic Health Record (EHR) all patients’ encounters daily.
- Enter charges at time of service for billing.
- Review and address tasks daily.
- Prioritize to achieve maximum productivity.
- Ensure patient confidentiality and EMR security at all times.
- Maintain patient information within HIPAA.
- Keep other care team members informed when situations occur that disrupts timely patient flow through site.
- Consistently participates in and actively adheres to patient care expectations to attain clinical goals set forth in the PCHG Health Care Plans and the Quality Improvement / Quality Assurance (QI/QA).

- Maintains a professional relationship and positive attitude with co-workers, the public, patients and the entire organization.
- Maintains the highest professional and work ethics.
- Displays enthusiasm toward the work, mission and vision of the organization.
PROFESSIONAL KNOWLEDGE, SKILLS AND ABILITES:
- Master’s degree in Nursing
- Graduate of an accredited NP program with a clinical specialty area of Family Medicine
- Current Florida NP certification and RN license.
- Minimum of three (3) years experience as an NP
- Current Basic Life Support (BLS) certification.
COMMUNICATIONS SKILLS:
- Exceptional customer service skills.
- Ability to communicate effectively, verbally and in writing.
- Excellent grammar, spelling and interpersonal skills.
