About the Case Manager Position:

The Case Manager for the Long-Term Foster Care, under the supervision of the Lead Case Manager, acts as an advocate for the youth, supporting them in exercising their rights, choices, and responsibilities. The Case Manager oversees the adaptation of the youth to the community and home environment, and the process of family reunification, as possible. This position requires the consistent exercise of discretion and judgment. The Case Manager is a grant position related solely to the Long-Term Foster Care Group Home as funded by the Office of Refugee Resettlement (ORR) and any successor funding.

About the Long-Term Group Home

The Long-Term Group Home (LTGH) is one of Maryville’s contributions to the safety and well-being of children globally. Boys and girls displaced by war, violence, abuse or chronic poverty find a home at Maryville when there is no family member to care for them. Maryville’s group homes for boys and girls ages 12 through 17 offer a trauma-informed environment that permits New Americans to develop their skills and talents in a community of support and safety. The Maryville Long-Term Group Home is federally sponsored through the Health and Human Services’ Office of Refugee Resettlement and ensures that the children’s physical, emotional, spiritual and psychological well-being is protected.

ESSENTIAL FUNCTIONS:

  • Coordinates and handle an estimated case load of eight long-term foster care residents at any given time, and provides coverage on additional cases, as needed;
  • Coordinates the initial intake for each child;
  • Actively participates in ORR staffing sessions, internal staffing, training and organizational meetings, and Treatment Team meetings;
  • Confirms that each child has an initial medical evaluation within 48 hours of arrival and coordinate with nurse health interventions;
  • Coordinates and assists the clinician in all required assessment procedures;
  • Serves as the liaison for the child in the community school and other settings;
  • Responds to client emergencies within or outside work hours;
  • Actively participates in ORR staffing sessions, internal staffing, training and organizational meetings, and Treatment Team meetings;
  • Upholds ORR Zero Tolerance of Sexual Abuse policy and Maryville’s Code of Ethics;
  • Manages reunification and post-18 processes for each case assigned;
  • Initiates and completes ORR-mandated procedures for vetting potential sponsors;
  • Keeps information updated in UC portal and in Maryville’s eVOLV system;
  • Coordinates the discharge and release of child to their sponsor, when approved by ORR, or to post-18 venue;
  • Fulfills ORR-mandated file requests;
  • Participates in Maryville File Utilization Review and eVOLV system meetings;
  • Contributes to the safety of LTFC youth at Maryville and when in the community, and responds to client emergencies;
  • Maintains professional relationships with youth, staff, community partners, stakeholders;
  • Other duties as assigned.

MINIMUM REQUIREMENTS:???????
Education/Experience:

  • Bachelor’s degree in relevant behavioral/social sciences field;
  • 2-4 years progressive experience in case management.
  • Skills /Knowledge:
  • Bilingual English/Spanish language competency required;
  • Strong communication and organizational skills;
  • Strong skills in Microsoft Office and web-based databases;
  • Strong sense of ethics and capacity to interact with ORR stakeholders;
  • Demonstrated willingness to be a team player;
  • Must possess a valid driver’s license.

  • Required to frequently sit, stand, bend, stoop and walk, sometimes for extended periods; able to walk up and down stairs;
  • Required to lift/move objects and/or youth weighing in excess of 40 pounds;
  • Must have visual, hearing, and learning capabilities sufficient to perform the essential functions defined above.

Environment and Scheduling:

  • Work is performed in designated program
  • Duties involve contact with youth who may be traumatized
  • Must be flexible and available to work additional hours when necessary
  • May be scheduled to work on weekend and/or holidays

All employees who have access to health information whose confidentiality is protected by the HIPAA Privacy Rule are required by the Rule to receive training so that they have an understanding and knowledge of the Privacy Rule that corresponds to their job responsibilities and of the group health plan’s policies and procedures that impact on their job duties. Site specific training on requirements of the HIPAA Privacy Rule and the plan’s health information policies and procedures under the direction of the plan privacy officer may be required for this position based upon site specific requirements. Employees who violate the requirements of the HIPAA Privacy Rule will be subject to discipline, up to and including termination.
