POSITION SUMMARY
The Youth and Family Recovery Support Coordinator will provide peer support and care coordination services to Transition Age Youth (TAY) ages 16 – 25 who have substance use disorder or display substance misuse, to ensure that they are engaged in treatment services and are connected to recovery supports in the community. Provides support, and advocacy assisting families with information, education, and linkage to community resources. Services are delivered in center or community-based settings depending on the needs of the families.

EDUCATION AND EXPERIENCE/QUALIFICATIONS
Must have lived recovery experience and experience supporting individuals in recovery, or a Bachelor’s degree in psychology, social work, or related human services field required. A valid driver’s license and access to reliable transportation is required. Candidate must speak fluently in English and Spanish.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide education on addiction recovery.

Assist, in conjunction with other team members, recovering persons to develop and implement their unique recovery plans, inclusive of the person’s personal interests, goals, strengths and vulnerabilities regarding recovery.

Utilizes Motivational Interviewing (MI) strategies to promote TAY readiness and action towards recovery.
Actively identify and support linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the recovering person’s goals and interests. Assists TAY in identifying and strengthening recovery supports within his/her family and or natural ecology.

Provides empowerment and advocacy for families, focusing on assisting families in developing effective partnerships with schools, providers and others to best meet their children and family’s needs.

Provides peer support and role modeling to teach families how to identify needed resources and supports in their communities.

Supports direct case management services and supports the development of case management plans specific to each assigned client and family, including interventions necessary to increase the probability of treatment completion, and to improve life functioning.

Maintains records of direct services for reporting and tracking purposes and completes all documentation for client record in accordance with documentation timelines and in compliance with Medicaid, licensing and other credentialing authorities.

Attends team meetings, staff meetings or rounds to assist in the development of case management needs and service plan review.

Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate.
Administers GPRA Survey to clients at multiple times across length of program involvement.
Assist Tree Program staff and treatment providers in implementation of program goals, data collection and care coordination.
Assists in professional training, outreach, and public education services as assigned. This may include professional conferences, workshops and training for behavioral health agencies. May represent the agency at community forums or serve on inter-organizational committees as directed.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
